Windows 11 Replacement Requests

Description

Information Technology is responsible for the technology procurement process. We are responsible for acquiring computer equipment for GVSU faculty and staff using university funds. Please utilize this form to request Windows 11 technology replacements. 

To request a Windows 11 replacement, click on the "Windows 11 Replacement Requests" button in the right-hand navigation menu.  Please note that this form is designed for one request per person and does not accommodate multiple user requests. 

Eligibility

This service is available to Faculty and Staff who've been selected for Windows 11 replacement. 

How to Access and Use 

Please take a moment to fill out the form by clicking on "Windows 11 Replacement Requests" to request a Windows 11 replacement machine. Please note that this form is designed for one request per person and does not accommodate multiple user requests. 

Instructions: 

Complete the Form: Fill out all the required fields, the more information you provide, the better!  

Submit a Ticket: After you’ve filled in the required fields, click Submit a Ticket.  

More information: If additional information or approval is needed, the ticket will be returned for more information or reassigned for additional approval.  

Response: Technology Acquisition will submit your order in Workday. 

Resolved: After Technology Acquisition places your Internal Service Delivery request in Workday, the ticket will be resolved.
